Please Note: All athletic activity has certain inherent risks. We want to insure that all players have a safe camp. Therefore, players will be expected to comply with all safety rules.

Camp will begin and end at the scheduled times. We request that players be picked up immediately after camp. No player will be allowed to leave camp early without permission from a St. Francis coach.

By checking here, I give my permission for a representative of the University of St. Francis and any doctor or medical facility to render any necessary treatment to my daughter (ward) in the event she is injured or becomes ill during my absence from the St. Francis camp.
