Video Request Form

The Center for Instructional Delivery (CID) can process a video for streaming within your online course. Upon receipt of this form and the video, CID will evaluate your request and will contact you with a timeline for completion.

CID requires a two-week turnaround to complete your request. Please be as specific as you can in your responses below.

Note:

A DVD may be copy-protected and therefore unavailable for your proposed use. It is unlawful to bypass an encryption method that protects videos from being copied.
All videos to be processed for streaming must be "lawfully acquired" (purchased or rented). In other words CID cannot use a copy.
