Registrar's Office - Requesting Transcripts
The Registrar's Office will furnish a transcript of credits completed to a student upon written request. Requests for transcripts must include payment of $5.00 per transcript. (Checks can be made payable to the University of St. Francis). Transcripts are not issued to or for students who have financial obligations to the University until those obligations are paid in full.
REQUESTING TRANSCRIPTS
Complete a TRANSCRIPT REQUEST FORM* and mail or fax it to us at the address/fax number below;
When requesting a transcript by mail or fax, include in your request:
- Your name
- Your curent address
- Your student number or social security number
- Your date of birth
- The approximate date of last attendance at USF
- Former name(s) you may have used at USF
- The address where transcript is to be mailed
- Your signature
- Appropriate payment of $5.00 per copy (faxed requests must be accompanied by a credit card number (Discover, Master Card, or Visa) and an expiration date
- Please include a day time phone number or email address so we can call if there are issues
- Instructions for when to send transcript:
- Immediately
- After current grades are posted
- After pending degree is posted
