Faculty Purchase Request Form

This service is available only to current USF faculty (full-time and adjunct). Orders will be processed as they are received; subject to approval of the Library Collection Development Committee and/or the Director of Library Services.

In order for the form below to be submitted, all required fields (marked with a *) must be filled out.
If some piece of information is not applicable or not known, put something in the box, even if it is only a "?". Also, please be sure to give a full email address (i.e. johndoe@nowhere.com). If you forget to fill out a required field you will get an error message. Just use the BACK button on the browser to go back and fill in the field, then re-submit your request. For video requests please type "111223333" into the ISBN field.

Single requests totaling $200 or more will be referred to the department chair for approval. Please allow six to eight weeks from initial request for books to be available for checkout in the library.
