This service is available only for current USF students, faculty and staff.

Fill in the form below to be added to the library patron database. This will allow you to check out books both from USF and from other I-Share libraries through the I-Share Catalog. Please provide all requested information. Incomplete forms will be processed after all completed forms are finished.

Use this form only if you are a distance student not able to come to USF to obtain an ID card. If you already have a USF Student ID card, but are not registered as a library patron, click here to register your barcode.
