JPDSP Sign-in Sheet (word document)

To assist ACI and OER (Federal Grant Evaluators) in assessing the work done by JPDSP, USF furnishes documentation of meetings and events held, attendees, and a brief description of the actual event. The main means of documentation is the use of the JPDSP Sign-in Sheets.

A sign-in sheet should be completed for each event, noting a brief Event Description, location, date, and time. Each participant should print their name as well as sign the sheet, note their birth day & month, and indicate the partner institution they represent.

Do not fill in the Activity Category #, Activity Name, or New blanks; these are for data entry use only.

The sheet can be submitted to Barb Ingold in the College of Education.

Please use this form if you are ordering materials for the professional development schools. The form can be submitted to Barb Ingold in the College of Education.


Through the JPDSP, University of St. Francis faculty and Partnering School faculty may apply for and receive Programming Project Requests for action research projects, professional development conferences, workshops, seminars, or materials that will further the mission of the JPDSP. Programming Project Requests will vary in size and actual amounts awarded will be based on need. Decisions made are at the discretion of the Programming Project Requests Approval Committee.
