Spirituality in Nursing Questionnaire

Dear registered professional nurse,

You are invited to participate in a study to learn more about the uses of spirituality by registered nurses and advanced practice nurses. This study is being conducted by an investigator from the University of St. Francis Health and Wellness Center, and College of Nursing and Allied Health in Joliet, IL, USA, and is supported by HRSA grant # 1D11HP07464-01-00. It will take approximately 20 minutes of your time. THANK YOU for contributing to nursing knowledge about this subject!

If you would also like to participate in a follow-up interview conducted via telephone, please contact the researcher at: dnardi@stfrancis.edu or 815-740-3861 and indicate your willingness to volunteer, and leave contact information. You are in no way obligated to do so. The interview will be audio-taped for transcription accuracy. You would be asked ten questions that you can answer in depth, if you like. The interview should take no more than fifteen minutes. No identifying information will be required.

Confidentiality
You are not asked to give your name or address. Information you supply will be recorded and analyzed using numerical coding, and results are reported in aggregate form to fully protect your confidentiality.

Nature of Participation
Participation in this study is completely voluntary. You can choose not to participate or withdraw from participation in this study at any time.
